Volkman Ischemic Contracture

Volkmann’s Ischemic Contracture Presentation

This presentation on Volkmann’s Ischemic Contracture, prepared by Dr. Zeeshan Chaudhary from the Orthopedic unit at Mayo Hospital Lahore, provides a thorough overview of this severe condition. Designed for a medical or clinical audience, it explains the irreversible muscle contractures that occur due to ischemia and muscle necrosis in the forearm, wrist, and hand.

The presentation begins with an introductory segment that highlights Volkmann’s Ischemic Contracture as a permanent flexion deformity resulting from delayed treatment of compartment syndrome. It effectively covers the critical aspects of pathophysiology, detailing how ischemic injury progresses to muscle necrosis and fibrosis, leading to the hallmark deformity known as the 'claw hand.'

Key topics include:
- **Etiology**: Primarily caused by untreated compartment syndrome, with other factors such as fractures, burns, and crush injuries.
- **Clinical Features**: Recognizable by severe pain, swelling, and the characteristic claw-like hand deformity.
- **Diagnosis**: Based on clinical examination and patient history, with imaging as a supplementary tool.
- **Treatment Options**: Ranging from early fasciotomy to prevent ischemia to advanced surgical interventions like tendon transfers for severe cases.
- **Prognosis**: Emphasis on early intervention to improve outcomes, highlighting that delays can result in permanent functional loss and chronic pain.

The presentation also touches on the prognosis and complications associated with delayed intervention, reinforcing the importance of timely treatment. Its structured approach makes it ideal for educating healthcare providers on recognizing, diagnosing, and managing this serious condition.

In summary, this presentation is a valuable resource for understanding Volkmann’s Ischemic Contracture and could benefit both medical students and practicing clinicians.

Measles outbreak in Kasur

Measles Outbreak in Kasur: Healthcare Promises Fall Flat

Measles outbreak in kasur.
Preventive health downfall.

In recent days, the district of Kasur in Punjab has been struck by a severe measles outbreak, resulting in the tragic deaths of five children within five days. In the village of Kamiannah Thath, near Pattoki, children named Yaseen, Shehzad, Zubaida, Zainab, and Nawaz succumbed to the disease. Parents attribute these deaths to the lack of timely and adequate medical treatment.

Last month, a similar outbreak in the nomadic settlement of Phool Nagar resulted in the deaths of three children. These incidents highlight the shortcomings and negligence of the health department, directly contradicting the claims made by the provincial government. The vision of Punjab Chief Minister Maryam Nawaz to deliver healthcare services at the doorstep seems to be failing in reality.

Parents have voiced their concerns, blaming the health department's incompetence and inattention for the rapid spread of the measles outbreak. Protecting children's lives should be a top priority for the government, and the loss of these young lives due to insufficient medical care is a deeply tragic matter.

Citizens are now calling on Chief Minister Maryam Nawaz to take immediate action to prevent such heartbreaking incidents in the future. The government must urgently review the performance of the health department and implement appropriate measures to ensure that healthcare services genuinely reach the public and safeguard children's lives.

Healthcare Services: Promises vs. Reality

The glaring gap between the government's lofty promises and the harsh reality has once again disappointed the public. The spread of the measles outbreak and the resulting deaths of children in Kasur have exposed the failures of the healthcare system. There is an urgent need for the government to translate its promises into action and take concrete steps to improve the healthcare system.

Providing basic healthcare facilities to the people of Punjab is a fundamental responsibility of the government. The spread of measles and the ensuing deaths are a stark reminder that much work remains to be done. The government should immediately deploy medical camps in the affected areas, launch vaccination campaigns, and raise public awareness about health issues.

The Way Forward

To control the measles outbreak, it is essential to improve healthcare facilities and ensure they are delivered promptly to the public. The government must prioritize health in its agenda and take swift action to enhance the performance of the health department. Protecting the lives and property of the public is the government's foremost duty, and any negligence in this regard is unacceptable.

Third Polio Case of 2024 Detected in Pakistan

In a concerning development, Pakistan has reported its third polio case of the year.

The case involves a young girl from Killa Abdullah district in Balochistan who tested positive for Wild Poliovirus Type 1 (WPV1). Additionally, WPV1 was detected in environmental samples collected from Hyderabad in Sindh, as well as Chaman and Pishin districts in Balochistan. Officials from Pakistan’s polio eradication initiative are closely monitoring the situation.

The Regional Reference Laboratory for Polio at the National Institute of Health confirmed that the virus was found in stool samples from a child in Darozai union council of Killa Abdullah. This child exhibited symptoms of paralysis on April 20. Genetic sequencing of the isolated virus is currently underway to better understand its origin and characteristics.

Polio eradication campaign pakistan

Efforts to combat polio remain critical, and vaccination campaigns continue to be essential in protecting children from this debilitating disease. Let’s work together to ensure a polio-free future for all.

Please note that polio is a highly infectious disease that mainly affects children under the age of five. It invades the nervous system and can cause paralysis or even death. Vaccination remains the most effective way to protect children from polio. If you have any other requests or need further assistance, feel free to ask! 😊

Protecting Children: Addressing the Measles Crisis in K-P

 Measles outbreak

In Khyber-Pakhtunkhwa (K-P), the recent measles outbreak has raised serious concerns about the health and safety of children, particularly in remote areas where access to vaccinations is limited.

Addressing the Measles Crisis
Vaccination of children

Parents like Bakhtwar Gul from South Waziristan are worried about the well-being of their children, as the lack of available vaccinators and resources hampers efforts to combat the spread of measles.

The Extended Programme on Immunisation (EPI) is facing challenges due to protests by vaccinators, who cite inadequate funding and support from the government. Hasan Ali, an EPI vaccinator, highlights the urgent need for increased resources to ensure effective routine immunization.

Dr. Muhammad Hussain, President of the Pakistan Paediatric Association KP-Chapter, emphasizes the importance of measles vaccination in preventing the spread of the disease. Dr. Hamid Bangash echoes this sentiment, advocating for higher vaccination coverage to protect children from measles-related complications.

However, the EPI's efforts have been hindered by operational difficulties and resource reallocation for COVID-19 vaccination campaigns. As a result, many children remain vulnerable to measles, highlighting the need for sustained support and investment in immunization programs.

To address the crisis, targeted vaccination campaigns are being conducted in affected areas, with plans for an upcoming initiative called 'Big Catch-up' to bridge the immunity gap in older children. Nevertheless, continued efforts and collaboration are essential to overcome the challenges facing the EPI and ensure the health and safety of children in K-P.

"Challenges in Polio Eradication: Thousands Decline Vaccination in Recent Nationwide Drive"

In a recent national anti-polio campaign, a significant hurdle emerged as over 63,000 parents opted out of vaccinating their children, posing a setback to the ongoing fight against polio eradication in Pakistan.

The refusal rates were notably high in the province of Sindh, where 38,749 parents declined the vaccination. This was followed by 20,392 refusals in Khyber Pakhtunkhwa (KP) and 4,081 in Balochistan. Moreover, smaller numbers of refusals were reported in Punjab, Islamabad, and Azad Kashmir.

The nationwide campaign, conducted from February 26 to March 15, aimed to reach children across the country. However, the Ministry of Health revealed another concerning development: the detection of the polio virus in environmental samples from eight cities. Among these samples, 12 contained traces of the virus, including one case of wild polio virus and three YBA cluster cases.

These refusal rates, combined with the presence of the polio virus in multiple cities, highlight the immediate need for heightened public awareness and education on the importance of polio vaccination in eliminating the disease from Pakistan.

Fasting and kidney disease patients: Ramzan,Holy month.

Ramzan and patients

Patients afflicted with kidney ailments or diminished kidney function risk severe complications if they undertake a month-long fast during the holy month of Ramadan without caution. Health professionals warn that individuals with reduced kidney function are especially vulnerable to serious, potentially life-threatening conditions if they fail to recognize the severity of their condition and fast without consulting their doctors beforehand. Kidney diseases, often dubbed as silent killers, encompass various conditions such as urinary tract infections, kidney cysts, kidney stones, acute kidney injury, and chronic kidney disease (CKD), which can be particularly perilous as symptoms may not manifest until significant, irreversible damage has occurred.

Research indicates that CKD is characterized by kidney damage or reduced function lasting more than three months. As kidney function declines, waste products and excess fluids accumulate in the body, leading to symptoms like ankle swelling, vomiting, weakness, poor sleep, and shortness of breath. Left untreated, these symptoms can progress to complete kidney failure, posing a serious and potentially life-threatening situation.

Experts stress the importance of individuals with CKD consulting their doctors and undergoing a comprehensive medical evaluation before embarking on fasting during Ramadan. Those with diminished kidney function should take precautions to avoid dehydration, a common cause of acute kidney injury, which can result in sudden kidney failure, a critical condition. Kidneys play vital roles in maintaining bodily functions, including filtering blood, regulating electrolyte balance, and controlling blood pressure. They also produce essential substances like renin for blood pressure regulation and active Vitamin D for bone health and stimulate red blood cell production.

Health professionals caution that individuals with type 1 or type 2 diabetes and hypertension are at heightened risk of kidney-related complications if they do not take extra precautions while fasting. Diabetes and high blood pressure are identified as leading causes of CKD. Individuals with reduced kidney function should promptly seek medical attention if they experience any complications while fasting. Those with urinary tract infections, mild kidney stones, or kidney cysts may be able to fast during Ramadan under medical guidance.

In cases of asymptomatic kidney stones, fasting may be feasible, but individuals experiencing pain due to stones should seek medical advice before fasting. Dehydration can exacerbate acute kidney injury and further harm kidneys. Likewise, individuals with CKD, hypertension, and high blood sugar may suffer irreversible damage if they fast without proper guidance from a qualified physician.

strawberry:A fruit for diabetes patients.

Strawberry 🍓 : A fruit for diabetes patients.

Diabetes is a significant global health issue impacting millions worldwide and leading to severe complications like cardiovascular problems, kidney damage, and nerve disorders. Type 2 diabetes (T2D) comprises about 90% of all diabetes cases globally, often influenced by lifestyle factors such as poor diet, sedentary lifestyle, and obesity. Pakistan particularly bears a heavy burden of diabetes, with around 19.4 million adults affected, ranking among the top ten countries with the highest prevalence.

Common treatments for diabetes involve anti-diabetic medications, which can have adverse effects like gastrointestinal discomfort and adherence issues. Hence, there's a growing need for innovative approaches to address this health challenge.

To explore potential solutions, a study investigated the effects of strawberry extracts on Alloxan-induced diabetic rats, focusing on their anti-diabetic, antioxidant, and antihyperlipidemic properties. Strawberries are known for their abundance of bioactive compounds, offering various health benefits. Results revealed that strawberries possess inherent health-promoting qualities due to their phytochemical content, antioxidant capacity, and nutritional richness.

The study suggests that consuming strawberry fruit extract could serve as a practical strategy for managing hyperglycemia and hyperlipidemia in diabetic individuals. Overall, these findings underscore the promising role of strawberry extracts in alleviating diabetes-related complications, advocating for healthier dietary choices.

B۔balance ... توازن برقرار) نہ رکھ پانا)

            (آنکھ کا مسئلہ)E.eye

F.face ( چہرے کی کمزوری)

A.arm(  بازو کی کمزوری )

S. speech ( بات کرنے میں مشکل)

T.time (  وقت پر ہسپتال پہنچنا)
